    Your active MC operating authority letter from the FMCSA.

    Completed and signed IRS W-9.

    Certificate of Insurance (COI)(required)

    Current COI listing Leo Dispatch Inc as certificate holder.

    Notice of Assignment (NOA)

    Optional — from your factoring company, if you factor your invoices.

  5. 5
    Voided Check

    Optional — a voided check for the bank account we'll use for payment.
